Fees and Payments
We accept cash, credit card and check. If applicable, co-payments are due at the time of service. For self-pay patients, we offer a 20 percent discount for treatments paid for in their entirety by cash, check or credit card on the day of service.

Appointments and Cancellations
We respect your time and strive to offer appointment times that fit into today's busy schedules. We provide evening hours, same day appointments, lunch-hour appointments and walk-in visits for cold, cough,flu and bladder infections. We estimate the time needed for your visit based on what you tell us about your problem so please tell our staff about all of your medical problems that you'll need to discuss.

We understand that sometimes you need to change your appointment time. We ask that you notify us 24 hours prior to your scheduled apppointment if you need to cancel for any reason so that we can accomodate another patient during that time. Please call ahead if you will be late. If you arrive late, please understand you may need to wait for a "work in" spot.

Insurance and Insurance Cards
Your health insurance card is just like a credit card. Please bring it with you to every appointment for any needed verification.

Please familiarize yourself with the rules and covered benefits of your insurance policy We do not know what is covered under your individual policy. Ultimately it is your responsibility to follow the rules of your insurance plan to ensure proper payment. Some insurance plans require a specific laboratory or hospital system for diagnostic tests so you must show us your insurance card at each visit. For other services, such as x-ray services, it may not be possible for us to know the least expensive facility for your particular plan so please cal your insurance company for this information.

Prescription Refill
Please call your pharmacy whenever you need a refill of medication. The pharmacist will contact us and your physician will review your chart and order refills if appropriate. Prescriptions will likely not be refilled if your physician has not seen  you within the past 12 months. Except for emergencies, prescriptions are not refilled after hours or on weekends. Please allow 48 hours for all refills.

Referrals
Most insurance plans do not require referrals but we ask that you please check with your insurance carrier prior to requesting a referral.

After Hours Call
Our providers are on call nights, weekends and holidays to manage your urgent health care needs. If you need to contact us after hours, call 303.673.9090 and follow the prompts to page the on-call provider  IN CASE OF EMERGENCY, CALL 911.
